Merit Mobile DNA - Policies and Procedures
At Merit Mobile DNA, we are committed to providing accurate, reliable, and professional DNA testing services while ensuring a seamless experience for our clients. Our policies and procedures are designed to uphold professionalism, confidentiality, and fairness throughout the testing process.
Appointment Scheduling
Appointments can be scheduled at any time. Once booked, customers will receive a confirmation via email or text to verify all details. We recommend scheduling early to secure your preferred time slot.
Mobile Service & Travel Fees
For appointments beyond a 20-mile radius of our office, a mileage fee of $0.67 per mile (round trip) will be applied. This ensures we can extend our mobile services while covering travel expenses.
Additionally, a $25 mobile convenience fee applies for on-site DNA testing within our service area.
Appointment Location Requirements:
• The testing location must be private, quiet, and accessible.
• It is the customer’s responsibility to provide a suitable environment for sample collection.
Cancellation & Rescheduling Policy
We understand that circumstances may change. To accommodate all clients fairly, our policy is as follows:
• Cancellations with at least 24 hours’ notice: No penalty.
• Cancellations with less than 24 hours’ notice: A $50 cancellation fee applies.
• No-shows (failure to attend the appointment without notice): A $75 no-show fee applies, and the appointment will be considered canceled.
• Rescheduling: Allowed at no additional cost if requested at least 24 hours before the original appointment. If rescheduling is required due to lack of proper identification, a $25 rescheduling fee will apply.
Refund Policy
• Once a DNA test has been administered, payment is non-refundable.
• If you cancel at least 24 hours before the appointment and testing has not occurred, a refund may be considered.
• No refunds will be issued for:
• No-shows
• Failure to provide valid identification
• Collected samples
Identification Requirements
All individuals undergoing DNA testing must present valid photo identification at the time of the appointment. Acceptable forms of ID include:
✔ Driver’s license
✔ State-issued ID card
✔ Passport
Failure to provide proper identification will result in a rescheduled appointment and a $25 rescheduling fee.
Results Timeline
• Standard DNA tests: Results available within 3 to 5 business days after sample collection.
• Legal DNA tests: Results available within 5 to 7 business days.
• Rush processing may be available for an additional fee.
Payment Terms
• Payment is required at the time of service.
• We accept credit cards, debit cards, and PayPal.
• Payments are non-refundable once the test has been administered.
Health & Safety Policy
To ensure a safe testing environment:
• We follow strict health and safety protocols during specimen collection.
• If you are experiencing symptoms of illness or have been in contact with someone who tested positive for a contagious disease, we will reschedule your appointment without penalty.
Additional Guest Policy
To protect privacy and confidentiality, only individuals directly involved in the test may be present during the appointment.
Exceptions require prior written approval.
